Please read this Electronic Notice and Consent to Share Information With Your Agency (the “Consent”) carefully and retain a copy for your reference. Defined terms contained in this Consent shall have the meanings assigned to them in the Cardholder Agreement between you and Sunrise Banks, N.A., unless otherwise defined in this Consent. For purposes of this Consent, "we", "us", and "our" mean Sunrise Banks, the issuer of the Card, and Debit Card Network, LLC, the servicer of the Card. "You" and "your" mean the Card Owner and any Authorized User of the Card. You understand and consent to us sharing certain of your Card Account information with the representative payee, conservator or public guardian (the “Agency”) that is authorized to load your Card Account by direct deposit. You agree we may share with Agency any personal or transaction information we obtain in connection with your Card or Card Account, including, without limitation, your name, address, telephone number, email address, Card Number, Account Number, or Card Account balance information and transaction activity.

You further acknowledge and agree that such information will be used by Agency to (i) monitor your use of your Card; (ii) add funds to your Card Account through direct deposit; or (iii) initiate a request to return funds from your Card Account to the Agency. Additional information regarding the Agency’s ability to add funds to your Card Account and to request a return of funds can be found in the “Adding Funds to Your Card Account” and “Return of Funds” Sections of your Cardholder Agreement. Except as otherwise set forth in this Consent, the Bank’s Privacy Policy and the “Information Given to Third Parties” Section of the Cardholder Agreement will govern the sharing of your personal information.

Withdrawal of Consent to Share Information with your Agency. You may withdraw your Consent authorizing us to share information with your Agency at any time by calling us at (866) 783-3248 or by writing to us at Debit Card Network, LLC, 2640 Cordova Lane, Ste. 105, Rancho Cordova, California 95670. You will not be charged a separate fee if you choose to withdraw your Consent; however, if you withdraw your Consent, you will no longer be able to load funds onto your Card, and you will not be able to use your Card after the existing balance in your Card Account is depleted. Any withdrawal of your Consent will be effective only after we have had a reasonable period of time to act upon your withdrawal.
